3rd Party Service Provider Registration Form

This form is to be completed by all 3rd Party Service Providers attending Vianney Venues halls, Queen’s Hall in Moortown or St Paul’s Hall in Alwoodley, on behalf of our mutual clients.  For example, Caterers, DJ’s, Hall Decorators and Bocuncy Castle providers. 

Timing – To be supplied as soon as possible but no later than 12 noon, 2 weeks before the event. This should accompany the Public Liability Insurance Certificates and PAT Test Certificates (with listing of items tested – pss/fail) by each 3rd Party Service Provider
